The person who designed the AsiaOne Unsubscribe Feedback Form seems to be either unable to think laterally or unwelcoming of non-desired feedback.

Content: Nil / Ads are not relevant / The ads do not appear on my email
Email: Nil / Received too much email! / My email acct. has changed
Work: Nil / Too much work. No time to check / Study more important. Emails later"
Note that "Nil" is not a valid selection, so either way you're left asking for ads.
Here're some suggestions for additional options, since that will presumably be easier than coding in a text box:
Content: Content is not relevant / Too many ads appear in the emails / Content is of poor quality
Work: Better things to do than read your boring (and possibly unsolicited) newsletters / More important things to do than read your boring (and possibly unsolicited) newsletters"
